Hi, I was Gerry's Case manager from Anthem. I looked forward to talking to Gerry , he was always upbeat and positive outlook on his prognosis and just life period. I always looked forward to our appointment calls and it was my pleasure to assist Gerry with his concerns and questions. He loved his family very much and expressed that on our calls. My prayers are with you and your family at this sad time. May God give you the strength and peace you need. If you need anything please don't hesitate to reach out to me. Again I am very sorry for your loss.

My prayers have been with the Hoffman family since I met them last week. You could feel the love and close-knit bond of the family in the hospital room that day. Thank you for letting Deaconess VNA Hospice help you through the final chapter of his life.
