Loving mother of Katy (Seth Falk) Jacobs. Proud grandma of Summer Rose. Very fond daughter of Michael and Veronica Zukowski. Dear daughter-in-law of Marianne and the late Don Jacobs. Dearest sister of Irene Borucki, Shelia (Ray Peters) Zukowski. Sister-in-law of the late Jim Jacobs, David (Pat) Jacobs, Judy (Bob) Wobick, Chris Jacobs and Jeanne Jacobs. Dear aunt of Beth Borucki, Mitchell Peters, the late Brenda (Bill Sullivan) Borucki, Lisa and Danny Wobick, Laura, Michael and Thomas. Great-aunt of Samantha Borucki and William Sullivan. Godchild of the late Frances Zukowski. Also survived by other family and friends. Visitation at the Funeral Home Mon., July 23, 4-8 PM, prayer service at 7 PM. Our deepest sympathies are with you and your family. We have very fond memories of our high school friendship with Annie and are truly saddened by her untimely passing.

May your hearts soon be filled with wonderful memories of joyful times together as you celebrate a life well-lived. Ann and I went to Bay View High School together and also worked together. She was a great person and will be truely missed.
