Carolyn Annette "Sis" Cook, nee Ducksworth, 69, of East St. Louis, Ill., born Dec. 30, 1938, in East St. Louis, Ill., died Thursday, Nov. 20, 2008, at Memorial Hospital in Belleville, Ill.
Carolyn was preceded in death by her parents, William and Henrietta, nee Powell, Ducksworth Jr.; and a sister, Barbara Good.
Surviving are her husband, Eddie W. Cook Sr.; three children, Rhonda Cook, Eddie (Linda) Cook Jr., and Lily Cook; six siblings, Janet Williams, Erickson (Carolyn) Ducksworth Sr., Muriel (Michael Sr.) Williams, Pamela Ducksworth, Denise Ducksworth and Daryl Ducksworth; one brother-in-law, Arthur (Alice) Stuart; five grandchildren, LaMarcus Boyd, Kenyetta King, LaChina (Montreal) McCoy, Demetrius Cook, Antione Cook and Vivica Cook; five great-grandchildren; two aunts; nieces and nephews; other relatives and friends including Gertrude Polk and Mozetta Freeman.

Visitation: Friends may call from 6 to 8 p.m. Tuesday, Nov. 25, 2008, at M.E. Officer & Sons Mortuary, 8501 West Main St., Belleville, Ill.

Funeral: Funeral Service will be held at 11 a.m. Wednesday, Nov. 26, 2008, at Pilgrim Baptist Church, 1124 Dr. M.R. Lemons Blvd., East St. Louis, Ill., with the Rev. Charles Williams officiating. Interment will be in Sunset Gardens of Memory, Millstadt, Ill.
