Constance Rosemary Ford (née Ozak), born February 18, 1945, in Cleveland, Ohio, passed on to her eternal home on August 24, 2025 in Glendale, Arizona, after her long struggle with a disabling illness.

Connie, as she was affectionately known, is survived by her younger brother, Phillip M. Ozak, and sister-in-law Maryanne. She was the doting Aunt of Joseph, Hily, Phillip, Jennifer, Frank, and Timothy, and a host of great-nephews and great-nieces. She was preceded in death by her parents, Theresa and Frank E. Ozak, her sister-in-law Elaine, her older brother Frank C. Ozak, and her sister-in-law Roseann.

Connie was a light in the lives of all who had the privilege of knowing her. Her time on this earth was one marked by love, compassion, and unwavering faith in God. Her passing has left a void in the hearts of all of her family and friends. She enjoyed working with arts and crafts, had a great love of cats, and never missed an opportunity to attend large family gatherings. Despite the physical disabilities she lived with for most of her life, she always shone with a positivity that was infectious, peppered with beautiful laughter that will forever echo in the minds of those who loved her. She was a Born Again Believer and member of the Church for the Nations. Throughout her life she was always actively involved with her local churches, including singing in the choir, and spreading the words of her faith to anyone with an attentive ear.

Connie I thank God for our friendship. We met in the choir at Phoenix First Asaembly Church. I will cherish the times we spent together, especially the Italian dishes you cooked in your apt. in Phoenix. Also, when you played your little keyboard and sang songs to the Lord. My husband and I would like to convey our condolences to her brother Phil and the rest of the family. God bless you all. We will see each other again one day Connie, my sister in Christ.
