Dallas Polk Obituary
Dallas Polk Sr., 56, of Ft Smith, AR passed away on December 10, 2020. He is survived by his wife, Annette Polk, children Dallas Polk II, Alysha George, Whitney Cook and Britney Haaser, brothers Duane and David Polk along with 5 grandchildren. Funeral service will be at 1:00 PM Wednesday December 16, 2020 at The Pentecostal Church in Ft Smith. Visitation will be from 5:00 PM – 7:00 PM Tuesday at Lewis Funeral Chapel 4817 Kelley Hwy in Ft Smith.
