Evelyn Hammack Obituary
She was born in Brooklyn, N.Y., on Dec. 13, 1953, the daughter of Richard and Mildred D. (Johnson) Nagel.
Evelyn was a member of St. Mary's of the Lake Church.
She received her nursing degree, with highest honors, from Upstate Medical Center in 1974 and her bachelor of science degree, nursing, from Syracuse University, magna cum laude, in 1976, then graduated with highest honors as a family nurse practitioner from Syracuse Community General Hospital in 1982.
Evelyn came to Plattsburgh in 1980 and worked as a nurse practitioner for Dr. Richard Murphy, Dr. Richard Lutinski, Dr. Robert Mullaney and for Fast Track at the CVPH Medical Center ER and was the Sexual Assault Nurse Examiner Coordinator at CVPH. More recently she was a nurse practitioner for Dr. Maurice Racine in Champlain.
Survivors include her significant other, Dr. Maurice Racine of Plattsburgh; three sons, David N. Hammack of Potsdam, Daniel N. Hammack of Potsdam and Ricky N. Hammack of Plattsburgh; one daughter, Lexsee N. Hammack of Brooklyn; and one brother, Roy Nagel of California.
Calling hours will be held on Friday from 2 to 4 and 7 to 9 p.m. at the R.W. Walker Funeral Home in Plattsburgh.
A Mass of Christian burial will be celebrated Saturday at 10 a.m. at St. Mary's of the Lake Church in Cumberland Head.
Burial will be in the spring in St. Mary's Cemetery in Champlain.
Memorial donations may be made to the Chazy Youth Hockey at Scott's Memorial Rink.

I am so sorry to hear of your loss. We worked together for a long time and had many memories. My mom and sister watched Lexi and she was a DOLL!!! Your Mom was proud of all of you and will watch over you always.
