Hazel was born June 7, 1930, in Idaho, Ohio, the daughter of the late Zennie Oscar, and Ada Matilda (Williams) Mustard. On April 26, 1947, she was united in marriage Leonidas Woolridge, who preceded her in death on September 20, 2007.
She is survived by three sons, Charles Edmond Woolridge (Tiny), Leonidas "Leon" Woolridge, Jr (Charlene), and Roger Lee Woolridge; eleven grandchildren LaDonna Eversole (Carlos), Angel Swain, Rebecca Clifford, Lisa Caplinger (Bobby), Charles Woolridge, Jr, Ronda Harris, John Boedicker, III, Rachel Meier (Jeff), John Woolridge, Jr, Michelle Woolridge, and Matt Woolridge (Tonya); and many great grandchildren, and great-great grandchildren.
She was preceded in death by her parents; her husband; one son, John Woolridge, two daughters, Donna Mae Anderson, and Patricia Rita Williams; two grandchildren, Michael Woolridge, and Phillip James Anderson, Jr; a great grandson, Michael Woolridge, Jr; five brothers, Raymond, Cecil, Herman, Bob, and Paul; and five sisters, Louise, Martha, Opal, Rosie, and Nellie.
Hazel was a member of Yankee Hill Community Church.
Funeral services will be held 11:00 a.m., Tuesday, September 30, 2025, at Boyer Funeral Home, Waverly, Ohio with Pastor Jack Moore officiating. Burial will follow at Woolridge Cemetery, Piketon, Ohio.
The family will receive friends 5:00 – 7:00 p.m. Monday, September 29, 2025, and 10:00 a.m. until the beginning of the service on Tuesday.
