Janice (LaMontana) Dvorak, age 79, a longtime resident of Frankfort, Illinois, since 1982, passed away peacefully on January 2, 2026. She was born on June 23, 1946.
Janice was a devoted and loving mother and grandmother. She is survived by her children: Richard (Carolyn) Dvorak Jr., Paul (Heather) Dvorak, Jennifer (Jason) Forsythe, and Anthony (Sara) Dvorak; her grandchildren Richie, Mary, and Abby Dvorak (children of Richard Jr.); Emily, Christian, and Austin Dvorak (children of Paul); Reese and Lucas Forsythe (children of Jennifer); and Koren, Francesca, Nicolette, and Jackson Dvorak (children of Anthony). She is also survived by her brother, Dominic LaMontana, and many beloved nieces and nephews.
Janice was preceded in death by her parents, Joseph and Florence LaMontana (Greco).
Born and raised on the south side of Chicago on South Paulina Street, Janice grew up surrounded by her many aunts, uncles, and cousins in a close-knit family environment. Family was always central to her life-constant, supportive, and deeply meaningful. She had a genuine love for life that shone through her eyes and spirit, and she cared profoundly for her children, always encouraging them to work hard, succeed, and be their best.
Memorial Visitation will be held on Saturday, February 7, from 10:00 a.m. to 11:00 a.m., followed by a funeral Mass from 11:00 a.m. to 12:00 p.m. at St. Anthony's Church, 7659 W. Sauk Trail, Frankfort, Illinois 60423.
