She was preceded in death by parents, Elizabeth (Betty) and Ivan (Pete) Floom; daughter, Janice K. Smith. She is survived by daughter, Jacqueline (Art) Smith Stiffler; son, Jeffrey Smith; loving grandchildren, Cody J. Foltz, Christy A. Green; siblings, Susie Radtka, Jody Roth, Alan Floom, Betty Mayle, Cindy (Greg) Cartiea, Lori Dow, Tim (Jeanie) Floom, Vicky Marcelli, numerous nieces and nephews and special friend Stacy Howell.
There will be no services at this time, a Celebration of Life will be held at a later date. Condolences and special memories of Phyllis can be shared with her family online at www.paqueletfalk.com
