She is survived by her longtime companion, Forrest Champion; daughters, LeAnn Tate and Sheena R. Phung; mother Ann Ousley; sisters, Shelly Johnson; Stacie Cambol, and Paula Hammonds; brother, Arthur Johnson; grandchildren, Elyse Tate and Jordan Bolla.
She was preceded in death by her father Arthur T. Johnson and brother, Richard S.A. Ousley.
