Where to begin? To her family know how grateful I was that Jan was part of my life I was absolutely blessed for 10 years h having Jan as my nurse practitioner she was fussy because she would tell me all the time all I wanted to see was her and not talk to McNally and she was right. Jan her warm smile. she represents the ideal of what a nurse practitioners are any medical provider should be she was always there to provide Stella medical care and counseling for my ailments I never forget when she introduced me to the VGO wearable insulin device I told Jan I could not do this she said yes you can and I did she had me even explain it to some other patient how easy it was and better for them. When ever I went for my appointment I will be in the office a half an hour or longer we were talking about Ryan and Christian then when Stafford came on board that´s all we were talk about showed me pictures of the wedding of Ryan and Kristian the grand children she was always preaching to me she wanted me to take care of myself wish my sugar would be running three and 400. So when she left out and didn´t come back they forward me to Kathy Berstein who also she´s just as sweet as Jan. I went in for my appointment on June 30 and I told K that I normally would send Jan Bible inspirational quotes but I hadn´t heard nothing in a long time and she told me Caroline Jane passed. I was lost for words I will always love her and miss her

We are so saddened to learn of Jan's untimely passing. She is irreplaceable. The world has lost a kind, caring, skilled, and compassionate caregiver, counselor, and friend.

The following letter to IMS a few years ago is a sterling example that typifies Jan's character and ability:

Dear Dr. ^^^^^^^^:
Where to begin? To say that we are very grateful to have Jan Sullivan on my medical support team would be a gross understatement! She represents the ideal of what a nurse practitioner -- or any medical provider for that matter -- should be. She has always been there to provide stellar, conscientious medical care and counsel for my varied ailments. However, as relates to my latest medical challenge, her actions can be viewed as literally lifesaving!

During a September visit, Jan's evaluation led her to prescribe a 48-hour Holter Monitor. Analysis of the monitor's results revealed that my heart had literally stopped beating for over five seconds. The evaluating cardiologist advised scheduling insertion of a pacemaker immediately.

This all occurred while Jan was on vacation. Jan called me on the Sunday night that she returned from vacation -- after studying my case (again, on a Sunday night). I advised her that the highly recommended cardiac surgeon was not available until November. Thanks to Jan, she was able to arrange for the surgeon to see her the following day, October 15 and to insert the pacemaker on October 16!

We are not aware of any recognition awards for nurse practitioners as there are for nurses. If there is one, we heartily and highly recommend Jan be recognized for her outstanding service to her many patients. The medical profession is most fortunate to have someone of Jan Sullivan's caliber among its ranks!

Sincerely,
Corinna Biondolillo

We are so fortunate to have had Jan as part of our lives for many years. Frankly, she has left a void that can never be fully filled.

Our prayers go out to her wonderful family with the hope that her memories can sustain them until they are all reunited.

Love, Prayers, and Blessings,
Mike and Rina Biondolillo

May her family know how grateful we are that Jan was part of our lives! As did so many of her patients, we considered her a friend -- not only a medical care provider.

May the wonderful memories shared with her family sustain them until they are reunited.

We offer our love, prayers, and blessings to her soul and her family,

Condolences to the family. Jan and I started as coworkers at Touro back in the late 1980´s later became good friends and once again coworkers in 2018 at IMS. I was absolutely blessed for knowing Jan. Her warm smile and kind spirit will be missed.
