LACON - Richard Forman, age 63, of 404 South St., Lacon, died at 10:10 a.m. Saturday, Dec. 4, 2010, at Proctor Hospital in Peoria.
He was born June 5, 1947, in Amboy, Ill., to the late Roy C. and Verna Mae (Holbrook) Forman. He married Regina A. Brown on May 10, 1969, in Lacon. She survives.
He is also survived by one son, Chris Forman of Peoria; one daughter, Kim (Tim) Mangan of Dunlap; and one grandson, Jacob Mangan, also of Dunlap.
He was preceded in death by one sister, Janet Cassidy.
He was a laboratory technician at Proctor Hospital for 36 years.
He served in the United States Army as a medical lab technician.
He was a member of Immaculate Conception Catholic Church in Lacon, where services will be at 10 a.m. Wednesday, Dec. 8, 2010, with Father Thomas Mizeur officiating. Visitation will be from 4 to 7 p.m. Tuesday, Dec. 7, at Lenz Memorial Home, 823 Fifth St., Lacon. Interment will be in Lacon Cemetery.
Memorials may be made to Immaculate Conception Catholic Church, the Lacon Food Pantry or Lacon Community Center.
Arrangements are by Lenz Memorial Home Funeral and Planning Services of Lacon.

Reg, I thought about you all night. I Know Danny and Rick found a guiding light in you. You as an older cousin were always an inspiration in no matter what you did... in music .your piano always played the tune your horse at the Henry County Fair always danced to the tune I am sure your co workers have seen what I have seen. I was shocked to see that Richard had passed away! I worked at Proctor in labor & delivery years ago when we were all young and enjoyed his so much. Regina I remember you too! My sympathy and thoughts are with you and your children.
