Leonard Ptak Obituary

Middletown, NY

Dr. Leonard Ptak of Middletown, died Monday, September 7, 2009 at Orange Regional Medical Center, Horton Campus, surrounded by his family.

He was born on October 24, 1926 in Greenville, NY to the late Anthony and Genevieve Reiss Ptak.

He was a Eucharistic Minister and a member of Our Lady of Mt. Carmel Church in Middletown, member of Neurological Association of Westchester, as well as an Honorary Physician of NY State Police. Leonard was also a member of the NYPD Emerald Society. He was a graduate of Palmer school of chiropractic medicine in Davenport, Iowa.

A family statement reads: "He was loved by all, blessed with talented hands; healing as a chiropractor, creative as an artist and imaginative as a chef and able to fix just about anything."

Leonard is survived by his wife, Susanne, at home; two daughters, Dr. Antoinette Ptak of Milford, DE and Lenore Ptak Sullivan and her husband, and Dr. Edward Sullivan of Canandaigua, NY; one brother, Walter and his wife, Loretta of Middletown; sister, Dr. Helen Ptak and sister-in-law, Frances Ptak both of Middletown; two grandchildren, Abigail and Quinn. He was predeceased by one brother, Henry.

Visitation will be held from 2-5 and 7-9 p.m., Friday, September 11, at Applebee-McPhillips Funeral Home, 130 Highland Ave., Middletown, NY 10940.

A Mass of Christian Burial will be celebrated Saturday, September 12, at 11 a.m. at Our Lady of Mt. Carmel Church with burial to follow in the family plot in St. Joseph's Cemetery.

In lieu of flowers, donations may be made in Leonard's name to the Building Fund for Our Lady of Mt. Carmel, 90 Euclid Ave., Middletown, NY 10940 and to the Fair Oaks Animal Shelter, 142 Bloomingburg Rd., Middletown, NY 10940.

I loved Dr. Ptak, he adjusted my back as a child, He was kind and gentle. I also learned never to ride my bike across his prized front yard. RIP

I am so sorry to hear the news. Dr. Ptak was the reason I became a chiropractor. For the many years he adjusted me, I became fortunate to experience his doctor / patient relationship as well as a friend and collegue. I carry a little bit of him with me in my practice today. He was an inspiration to me and my entire family. I will surely miss him.

I moved from Middletown in 1999 but still remember you and Leonard as the ever- faithful EM's at OLMC Church. I am so sorry to read of Len's passing. Please know my thoughts and prayers are with you. I bet Msgr. Schroeder was one of the first souls to welcome Len into God's Heavenly Kingdom! May God keep His healing hand on you and give you peace and comfort.

Dearest Sue, Len was an inspiration to all. He loved his family and was deeply proud of both his daughters and adored his grandchildren. As others have said, he always had a joke to make you forget your back pains and was a genuine healer. I was his patient for 33 years and adored him. I miss him already. He is with the Lord now, and no longer in any pain. Be strong for one another and know Len is watching you all and smiling.

Dr. Ptak made my back feel better. More importantly, he lifted my spirits. We shared a lot of laughter, over the years. I once fell through his adjusting table. You had to be there to see how funny it was.
