Carolyn Ann Provenzano, 45, of St. Augustine, Fla., died May 28, 2009, at her home. She was born in Jersey City, N.J., and had resided in St. Augustine for the past five years. She was a pre-school teacher and a member of San Sebastian Catholic Church.
Funeral Mass will be celebrated at 6 p.m., Saturday, May 30, 2009, the Rev. Thomas Walsh, pastor, officiating. Graveside services will be held 10 a.m. Monday at San Lorenzo Cemetery, the Rev. Dr. James Swanson, officiating. Flowers are gratefully declined those wishing may make a contribution in her memory to the Donna Hicken Foundation, 1015 Atlantic Boulevard, Suite 144, Atlantic Beach, FL 32233.
She is survived by her husband, Daniel; sons, Andrew and Daniel Provenzano, Jr; all of St. Augustine; her mother, Vera Collins; and brothers and sisters.
Craig Funeral Home Crematory Memorial Park is in charge of the arrangements.
