Dr. Jay Chuck, 82, of University Park, Florida died surrounded by his loving family and friends, November 2, 2015 at 6:12 pm.
Dr. Chuck was born February 27, 1933 in Warren, Ohio, son of the late Tracy Jay and Hazel Hendricks Chuck.
He was a 1951 graduate of Boardman High School in Boardman, Ohio. Jay was an outstanding high school football star; elected all state running back and inducted into the Boardman High School Football Hall of Fame in 1985.
He was awarded a full football scholarship to University of Cincinnati and received his Bachelor of Science from Mount Union College in Alliance, Ohio. Dr. Chuck then received his Doctorate of Medicine at Kirksville College of Osteopathic Medicine in Kirksville, Missouri in 1961.
Jay served in the Navy at the U.S. Naval Hospital in Bethesda, Maryland. Dr. Chuck was a dearly loved family physician with private practices in New Middletown and Hanoverton, Ohio and was on staff at the Youngstown Osteopathic Hospital from 1962-1988.
Following his retirement in 1988, because of his love for medicine, he continued emergency room medicine with Locum tenens and later worked for the Ohio State Bureau of Workers Compensation. Much of his retirement was spent between Ohio and Florida with his beloved wife, Ellie, where he enjoyed family, friends, travel, golf, tennis, and being together.
He is survived by his wife, Eloise (Ellie) Parish whom he married November 29, 1974. He leaves four daughters: Debra Janis (late Richard) of Boardman, Ohio; Cheryl Chuck (Brian Hodge) of Struthers, Ohio; Cathy Held of Canfield, Ohio; Lori McCreery (Jeff ) of Poland, Ohio. Six grandchildren: Jennifer Janis of Boardman, Ohio; Kristen Held of Canfield, Ohio; Adam Valko of New York City; Steven Held of Salem, Ohio; Blaine and Braden Griffiths of Cincinnati, Ohio. One great grandchild: Hailey Anglin of Boardman, Ohio. Siblings: Katherine Lugibihl (Edison) of Boardman, Ohio; Gerry Chuck (Gail) of Harrison, Ohio. Brother-in-law; Sal Parish of Boardman, Ohio and sister-in-law; Charlotte Smith of Cleveland, Ohio and many nieces and nephews.
Because Jay was a man of deep faith, his character evoked a presence of strength, compassion, honesty, and integrity. Our hearts are filled with not only sorrow, but with overwhelming joy to know the words he will hear 'Well done good and faithful servant!' Matthew 25:21
Dr. Jay and Ellie are active members of the Tabernacle Church of Sarasota, Florida. A Memorial Service will be held at the Tabernacle on DeSoto Road at 11:00 am, on Friday, November 6th. A memorial service will follow in Ohio at a future date. Any donations may be made to the Tabernacle Church Building Fund 4141 De Soto Road, Sarasota, Florida 34235.
