JUST HAD OF THE PASSING TO GLORY OF A GREAT MAN. DR. BLAINE CUSACK WAS A GOOD FRIEND OF MINE AND MY FAMILLY. THIS IS A SHOCKING NEWS TO US AFTER HEARING OF HIS DEATH TODAY. MAY ALMIGHTY GOD BE WITH ALL THE PEOPLE HE LEFT BEHIND AND GIVE HIM ETERNAL REST AS WELL. SLEEP WELL MY DEAR FRIEND.

I was thinking of you, Phyllis, and Blaine, and seeing you at the stairs. I am so sorry for your and your family's loss. Blaine was a wonderful, great guy. Always positive, and a true family man.

Blaine was my dentist, client, and friend for 33 years. Graduated a couple years ahead of me from Fenwick. A better guy you couldn´t find. I´ll sorely miss him.

I have been a patient of Doctor Cusack for several years, and we had great camaraderie, love, and respect for each other. He never failed to let me know that every time I visited. It was with real sadness that I learned of his passing. He was a good and loving man and will be missed.

Dr Cusack was a special doctor who really cared and made a huge positive impact on my life. He is sorely missed and lives on forever in the spirit of all the patients he helped and loved. The feeling of love and gratitude was always mutual for his outstanding talent and willingness to share.

I am devastated to hear of Dr.Cusacks passing. I´ve been his patient for more than 5 years and I´ve never met anyone like him. He was sincere, his energy was pure, and he was beyond thoughtful! What I´ll miss the most is his empathetic nature and gentle disposition. Even after his diagnosis he worked like a champ, rarely ever complaining. Doc was the best!!! He was so thoughtful and caring! He remembered the smallest things about his patients. I´ll be forever grateful to have met him....
