JUST HAD OF THE PASSING TO GLORY OF A GREAT MAN. DR. BLAINE CUSACK WAS A GOOD FRIEND OF MINE AND MY FAMILLY. THIS IS A SHOCKING NEWS TO US AFTER HEARING OF HIS DEATH TODAY. MAY ALMIGHTY GOD BE WITH ALL THE PEOPLE HE LEFT BEHIND AND GIVE HIM ETERNAL REST AS WELL. SLEEP WELL MY DEAR FRIEND.

I have been a patient of Doctor Cusack for several years, and we had great camaraderie, love, and respect for each other. He never failed to let me know that every time I visited. It was with real sadness that I learned of his passing. He was a good and loving man and will be missed.

I am devastated to hear of Dr.Cusacks passing. I´ve been his patient for more than 5 years and I´ve never met anyone like him. He was sincere, his energy was pure, and he was beyond thoughtful! What I´ll miss the most is his empathetic nature and gentle disposition. Even after his diagnosis he worked like a champ, rarely ever complaining. Doc was the best!!! He was so thoughtful and caring! He remembered the smallest things about his patients. I´ll be forever grateful to have met him. Dr.Cusack helped me tremendously when I had some of the worst dental emergencies. He was a God send. I´m sure he has a place in heaven. It´s disheartening to know that we will not see him until we cross over, but it´s pleasuring to know that I´d been graced with his presence. My prayers, love, and condolences to his family and close loved ones. May God rest his soul and provide peace for you all!

My dad was my biggest fan, the best cheerleader, and toughest man I know. He is the man who holds it all together. The man you go to when something breaks and tells you that´s easy to fix even if it takes you all day. Even the man to go to when you make a big purchase because his negotiation skills are like no other. He´s the one.
Dad, you and mom have given us the most amazing life and the best childhood. I´d do anything for one last hug or to hear you tell me how proud you are of me. Thank you for being the best dad a girl can ask for and grandpa to my little girls.
I´m honored to be your daughter and you will always be my hero.

I was one of Dr. Cusack´s patients ever since I was a small kid. I´m 25 now. Most kids avoid the dentist - I honestly looked forward to it, because Dr. Cusack was truly a role model in my life. He made an impact on me every time I saw him. He gave the best life advice, saw life through a lens of resilience and humor, and just cared. He exuded such love, more than most people ive met in my life.
His impact will stay with me forever. May he rest in peace. God bless him and your family. The Casini family is praying for your family.

I got to know Mr. Cusack from swimming with his daughter, Ashley. She and I became very close and I was lucky enough to hang out with the family on occasion. Mr. Cusack was always so full of life and usually had me laughing with his stories, as he never needed to sugar-coat anything. Beyond that, I got to see Mr. Cusack's unending love for his family. He loved playing in the pool with his kids and grandkids and making sure everyone around him was having a good time. His smile always showed in his eyes!
He once was kind enough to bring me along to a HAWKS game and it was a BLAST! He made sure I had the best time and was adamant that we have steak for dinner! It was the best!! But, beyond the fun of the game, something else stayed with me. Mr. Cusack had some patients who were at the game as well. The amount of time he spent with them spoke volumes to me. It was so clear that he truly cared for his people; family, friends, patients...anyone. He wanted every person he knew to feel loved. He will be missed by all of us, but his spirit and joy lives on through his family.

Dear Cusack family,
Our hearts are so heavy and deeply saddened. We could not have asked for a better dentist and friend. My kids knew they were loved and cared for when they saw Dr Cusack. They loved him so so much. He always remembered everything about them and he truly cared. We all have lost such a dear friend. There is no one like him. No one. I thank God that we were so blessed to have Blaine in our lives. We will never forget him and the impact he had on our lives.
Blaine, May our Lord Jesus hold you tightly and give you the biggest bear hug welcoming you to your eternal home. You ran the race. You made it my friend. Praise to you Lord Jesus Christ for your gift of salvation!!
