Mark H. Fields, Jr. M.D. 83, passed away on Monday, Aug. 16, 2021 at Advocate Condell Medical Center in Libertyville. He was born Sept. 4, 1937 in Sioux City, Iowa and had been a Libertyville resident since 1959. He was a graduate of the University of Iowa and served as a physician in Libertyville for 45 years. Dr. Fields was a member of the United Methodist Church, the American Academy of Family Practice, the American Medical Association and was a veteran of the US Navy. Surviving are his loving wife of 60 years, Mary Fields; 3 daughters, Patricia Fields-Perry, Catherine Fields and Helen Fields; 3 Grandchildren, Justin, Alyssa and Collin; his sister, Joanne Fields-Whitaker (David) and his brother Robert Fields. He was preceded in death by his parents, Mark Sr. and Eugenia Fields, his son Mark Fields, III and his brothers, Phillip, Henry and Gerald. A visitation will be from 3 to 8 pm Friday, Aug. 27, 2021 at the Burnett-Dane Funeral Home, 120 W. Park Ave. (Rt. 176, one block west of Milwaukee Ave.) Libertyville. A service will be held during the visitation at 7:00 pm, followed by military honors. Dr. Fields was a special human being. Beyond his professionalism, he truly cared about his patients. His caring went beyond the practice of medicine, it was nurturing in the best possible way. He left a lasting message for those of us that he touched and he will never be forgotten.
My prayers of peace for his beloved family and a lifetime of fond memories. Thank you for all the years of sharing this gifted man. Dr. Fields and Dr. Dolan were our family doctors since 1959. May they both rest in God´s good grace.

Dr. Fields, was a truly caring individual. He became my Doctor after Dr. Dolan passed away.
Dr. Fields, over the years took care of me entire family. Dr. Fields was one of a kind, a great doctor and a caring friend.

Dr. Fields was an avid scuba diver. He would scuba dive with his son, Mark, a close friend of mine. Great memories of being in their living room, viewing photos of their dives.

I want to thank your family for sharing your husband and father with my whole family. The Joergers and the Permanns. He saw us thru so many cirisis in our lives and we always new that he would protect us when in the hospital and new we had a Dr and friend looking out for us.
