PHYLLIS HAZELETT Obituary
PHYLLIS R. HAZELETT, 91, of Newark, Ohio, widow of Ted Hazelett, died April 6 at her residence. Funeral service will be 2 p.m. Saturday at Chapman's Mortuary; burial at Highland Cemetery. Visitation will be two hours prior to the service. A viewing will be held 5 to 7 p.m. Thursday at Vensil and Chute Funeral Home. In lieu of flowers, donations can be made to Generations Hospice. www.chapmans-mortuary.com.

Ms. Phyllis was a beautiful lady , kind and gracious.
