Dr. Dan Faulk Age 54, of Indianapolis, passed away March 26, 2010. He was born in Indianapolis to Lawrence and Betty Faulk and they survive. Dr. Faulk was an Oral Surgeon with Indiana Oral and Maxillofacial Surgery Associates for the past 25 years. He was a 1974 graduate of Ben Davis High School, 1978 Graduate of the University of Notre Dame, 1983 Graduate of Indiana University School of Dentistry, completing his Indiana University Medical Center Oral Surgery Residency Program in 1986. He was a Diplomat of the American Board of Oral and Maxillofacial Surgery, holding many professional memberships some of which are with the Indiana and American Dental Association, State of Indiana General Anesthesia, American Association of Oral and Maxillofacial Surgeons, Indiana Society of Oral and Maxillofacial Surgeons and the Indianapolis District Dental Society. Some of his many past and present professional appointments include; Assistant Professor of Oral Maxillofacial Surgery, Indiana Dental School Department of Oral and Maxillofacial Surgery, Delegate of Indiana Dental Association General Assembly, Chairman of Oral and Maxillofacial Surgery Dental Section at Methodist Hospital, President of Indiana Society of Oral and Maxillofacial Surgeons and President of Indianapolis Westside Dental Study Club. Dr. Faulk 's other memberships include; St. Monica Catholic Church, Assistant Scout Master- Boy Scouts of America, Speedway Optimist Club, Indiana Environmental Council, IU Alumni Association, University of Notre Dame Alumni Association-Founder's Circle Member, Hutton Circle Member at IU School of Dentistry and the Eagle Creek Foundation and Sailing Club. Dr. Faulk's practice was located in Speedway with a satellite office in Lebanon. His dedication to his patients and referring Doctors was a reflection of his love of Oral Surgery. Dr. Faulk's sense of humor, love of nature and positive attitude were a constant source of inspiration to his family and friends. He had many interests outside of his practice including; traveling, sailing, hiking, skiing and running. He will be missed and adored forever. Along with his parents, other survivors include his loving wife of 33 years, Janna (Gehl) Faulk; three children, Ryan(27), Riley(24) and Kelsey(23); three sisters, Gaynelle Rogers Mick (Jim), Paula Faulk Stout (Alan) and Deana Faulk Thomas (Jeff) and many loving extended family members. Family and friends will gather from 4:00 p.m. until 8:00 p.m. on Tuesday, March 30 at the Leppert Mortuary, Nora Chapel 740 E. 86th Street, Indianapolis.
