Craig Polivka Obituary
Craig is survived by his parents, James and Susan Polivka; brother, Steven (Laurie) Polivka and sister, Sarita (Scott) Schraeder and many beloved nieces, nephews and cousins.
Craig is preceded in death by his paternal grandparents, John Polivka Sr. and Estelle and maternal grandparents, Vernon and Gladys Slama.
A celebration of life will be held at a later date. Haase-Lockwood & Associates Funeral Homes and Crematory of Elkhorn, WI, is honored to serve the Polivka family. Online condolences can be made at www.haaselockwoodfhs.com
