Lorena Hernandez Carmona passed away peacefully on November 19th, 2023, in Livermore, CA, with her mother, Saralinda Lewis, and sister, Carmen Grundy, at her side. Born on 5/11/1977 in San Jose, CA, Lorena was a beloved and cherished member of her community, both near and far.

Lorena was known for her sense of humor, honesty, and above all, her fierce loyalty. She touched the lives of many with her kindness and generosity, especially within the underserved communities of the Bay Area.

Lorena will be deeply missed by her loving family members, friends, colleagues, and those in the community she served. She is survived by her mother, Saralinda Lewis, and father, Jesus Hernandez; her daughter, Isabella, and son, Isaiah (Jacqueline); the apple of her eye, her grandson, Isaiah Jr.; siblings, Augie Sanchez (Linda Masterson), Carmen Grundy (Richard), Bianca Valencia (Kevin), Celia Hernandez, Miguel Hernandez, Sandra Hernandez, and Jose DeJesus Hernandez. She held a special place in her heart for nephews and niece, Israel, Carmina, Aidan, and Ethan.

She was preceded in death by her brother, Tomas Hernandez, and her grandparents, Carmelita Aguilar, Albert Lewis, Victorina Torres, and Miguel Hernandez.

A celebration of life to honor and celebrate Lorena's life will be held on Saturday, March 23rd, at Bethel Family Christian Center, 501 N P St, Livermore, CA 94551, from 12:00 PM to 3:00 PM. All who knew her are welcome to attend and pay their respects.

In lieu of flowers, donations may be made to Roots Community Health Center or The National Pancreatic Cancer Foundation in memory of Lorena Hernandez Carmona.

Lorena's legacy of kindness, commitment, and advocacy as a Community Health Worker will continue to inspire and resonate with those who had the privilege of knowing her.

May she rest in peace.

Lorena was my friend when I was 13 years . I met her when I started 9th grade . She was so funny I remember her jokes made me laugh harder than anyone I ever met.. she always had a way of making anyone laugh.. she was a very good friend ,the type of friend who would protect you if you needed it, even if she thought you did even if it wasn't that situation she was a protective friend .. she wouldn't let anyone make fun of me or treat me wrong . I am saddened to hear she passed away.. over the years I always think of Lorena and her robust energy and smile, funny loving attitude . I will miss you Lorena , I just found out last night she passed away I'm sorry I wasn't there to pay my respects.. I have lost touch with her for some many years now and it's sad to know she gone... I love you Lorena Hernandez.. my name is Alexis ..

Lorena was my manager at Roots back in 2021-2022. She was warm, welcoming, and such a gregarious and funny person. We dealt with some serious situations and Lorena still kept such a sense of humor. She always told us to "meet people where they're at" -- with humility, without imposing our own expectations. She was able to build trust with people so easily because of this approach. I'll always remember her distinctive boisterous laugh. I know Lorena loved many and was loved by many; may her memory be a blessing.

Lorena and I worked together at a startup for Community Health Worker-led care. She was a guiding light for all of us. She served as both the heart and head of our team, instilling her empathy and knowledge to help us consider things we never would have thought of.

Lorena´s perseverance and steady presence has helped countless of us to learn, love, and serve better. She´s an inspiration to anyone looking for a way or a reason to help others. May her love and encouragement live on in the work that she started and that we may continue.

All our love to Lorena´s amazing family, and the Roots Community Center and all the folks whose lives were touched by Lorena.

I met Lorena in 2016 as I was facilitating a group called pathways to change. Lorena was the only person to show up on the first day so I had a chance to talk to her. She shared her dreams, barriers, success´s (Isabella) and her dreams. She said she wanted to do my job. I was taught a good leader helps to foster other leaders not followers. Lorena already had that quality as we began to co-learn and co-teach each other along the way. To remember that day to where she has gone has been an amazing thing to witness... will keep our agreements and continue to teach and inspire others... love you!

I had the pleasure of witnessing Lorena grow beyond her own expectations. Lorena was the only student that showed up for a group I was facilitating at Roots Community Health Center 2016 pathways to change. We spoke for two hour straight where she shared her frustrations, barriers, blessings and dreams. We became like brother and sister. I am fortunate to have been her facilitator and mentor, then she began to mentor and facilitate my growth. She called me on my BS and challenged me to grow. Lorena will always be with me as she has taught me so many valuable lessons. She also has blessed me with an extended family. Rest in Power my sister! It´s an honor to know and meet an angel here on earth, now you will blessed those that will call it a miracle. You are an inspiration and I will always love and cherish you!

The first time I met Lorena was when I was facilitating and class and she was the only one who showed up on the first day. This was a perfect example that Lorena spirit of being about it rather than talking about it. We spoke for two hours straight and she told me she wanted to do what I was doing, my response was your so much more than I will ever be! I witnessed, first hand her be presented with challenges and continuously strived to overcome. Lorena´s passion was to help people, and that she did! My sis will always be in my heart, spirit, and motivation for this work. Rest in Power! Your lessons will continue to teach and live with those who were blessed to be in her presence!

I had the privilege of meeting Lorena back in 2018 through Tim Berthold and the leadership team of the City College of San Francisco CHW Certification program. I was immediately struck by her quiet strength, humility, and passionate advocacy for the essential work that Community Health Workers do. Years later, I had the extraordinary honor of partnering with her on my latest start-up where she was our founding Community Health Worker Lead. Over a year and a half of closely collaborating together, I witnessed firsthand her insatiable thirst for learning, dedication to sharing her lived experience and ample knowledge with her teammates, and her unstoppable drive to make the world around her better. Meeting her all those years ago was a transformational moment for me. Most importantly, she taught me the power of resilience, despite seemingly having so many of the cards of life stacked against her. She is one of the purest of heart and soul that I've ever met and I often reflect on the lessons that she taught me as inspiration to be the best version of myself. May she Rest In Power and her legacy live on through others for eternity.

Sharing my prayers and strength with her beloved family during this difficult time. Know that you have a village around you if you ever need the support.

My first memory of Lorena, was the first day I started working for Roots Community Health Center, I need assistance with the Navigation and how the program works. So I asked her if she could help me. That day was the beginning of a solid friendship, I must say she knew her stuff and didn´t mind helping when help was needed.
As our friendship grew, I later became her Hair Stylist. Which was some of the best times spent with her, I think I laughed the whole entire appointment. Lorena had a beautiful Spirit. She also brought her beautiful daughter Isabella (very sweet little girl) . I will forever hold the memories we shared in my heart, she is truly missed.

I first met Lorena when she was a student in City College of San Franciso's Community Health Worker certificate program. I was the instructor for one of the certificate's required courses.

Lorena was quiet at first, so I really had no idea what compassion, kindness, and expertise lay inside her calm exterior. I came to learn that part of what made Lorena quiet was her absolute humility. She was already a highly skilled Community Health Worker, but she always made room for the experiences and points of view of the other students. She showed all of us how much could be learned by listening rather than by insisting on being the expert in the room. Lorena *was* an expert, but she never, ever saw herself as having more expertise or being better than anyone else. I came to rely on her skills and wisdom in class. I always knew that I could count on her to show leadership and insight, along with compassion and patience with her clients and with her colleagues.

I am still quite stunned that she is no longer with us in physical form, but she has influenced my life and the lives of so many others in ways that ensure that she will always be remembered. Thank you, Lorena. Your grace will forever be in my heart.

I first met Lorena as a student in the Community Health Worker Certificate (CHW) class I was teaching at City College of San Francisco. She impressed me immediately with her warmth, wisdom, and wonderful sense of humor. Lorena quickly became a leader in the class; she was someone who others turned to for encouragement and support. After a year of studying together, it was no surprise that Lorena was nominated to be the graduation speaker for her class.

Lorena and I stayed in touch as she served on the Advisory Board for the CCSF CHW Program. Later on we worked together as consultants training CHWs in Monterey County. Lorena was a dynamic trainer who had a knack for storytelling and a gift for communicating her respect and empathy for her students.

In 2023 Lorena and I had the opportunity to co-author several chapters for a book about the CHW profession. We worked together for 9 months writing, reviewing and editing our chapters. Writing with Lorena made what can often be a tedious process joyful.

Lorena was a friend, a colleague and a mentor to me. She taught me about public health and how to train CHWs and, most importantly, she taught me how to face adversity in life with courage and lovingkindness.

Thank you Lorena for being in my life. I love you!
