Kimberly A. Kozak, D.O.
North Myrtle Bech
Kimberly Ann Kozak, D. O., age 48, of North Myrtle Beach, passed away suddenly on Saturday, September 29, 2012.
Born on March 12, 1964 in Hazard, Kentucky, Kim is survived by her loving husband, Ralph Bradley Kozak, and her precious daughter, Kayla Nichole Kozak, age 13; her mother, Ruth Ann Spencer of Michigan; brothers Elmer Cornett and Donald Wayne "Skipman" Spencer, both of Michigan.
She earned her Bachelor of Science in Bio physiology at Eastern Michigan University, graduating Magna Cum Laude, and her Doctorate of Osteopathic Medicine from Michigan State University. Kim interned and completed her five years of General Surgery, Otorhinolaryngology and Oro-facial Plastic Surgery residencies at Oakland General Hospital in Madison Heights, Michigan. Board certified in both Ear, Nose and Throat and Facial Plastic Surgery, she was a member of the American Medical Association and the American Osteopathic Colleges of Ophthalmology and Otolaryngology Head and Neck Surgery.
Kim was employed by McLeod Health as an ENT at Seacoast Hospital in Little River, South Carolina. Prior to moving to South Carolina, she was in private practice in Grants Pass, Oregon. Kim attended Barefoot Church in Ocean Drive, and she enjoyed spending time with her family, music, theater and travel.
A celebration of life will be held Thursday, October 4, 2012 at 5:00 p.m. at Lee Funeral Home, Little River, South Carolina.
In lieu of flowers, memorials may be made to Barefoot Church KidZ Ministry, 701 Main Street, North Myrtle Beach, South Carolina, 29582.

Kim and I were very good friends back in High school, she was such a sweet person, Kim will be greatly missed by all that new her. She had such a beautiful happy and loving personality. Dear Ralph and Kayla, I'm so sorry to hear about Kim. She was a dear, dear friend to me when she was in Med school in Madison Heights. I still remember her apartment at the hospital. We grew to be close friends, a relationship I have always treasured. I stood up in your wedding in Canada and was so sad when you both moved to Oregon. I just found out about Kim's passing. I am so sorry for your loss. Kim was my roommate in college and stood up in my wedding. She was one of the sweetest person in the world. Beautiful both inside and out. Though we lost touch over the last several years, I will truly miss her. I am so sorry for your loss. She was a great Dr. My son was jumped by a bunch of Cage fighters a few years back and she saved his eye. She was very gentle and a great doc for my son. I talked my mom into seeing her as well because of ear problems and she gave my mom a better way of life. She will be greatly missed by my family. I thank God for her being in our lives.

I Worked with Kim at my mother and fathers restaurant back in Ypsilanti when she was in collage.
Kim was well liked by all who knew her including our entire family. I and my family are deeply saddened to hear of Kim's passing. Her memory will live on in all who knew her as Kim was a very kind and decent soul.

Thank you Kimberly for helping me through my serious allergy issues and caring surgical procedure to help me function again. Dr. Kim saved Vern's life by guiding him through cancer treatments with great knowledge and personal caring. We loved her as an accomplished physician and surgeon as well as a personal friend. We didn't want to see her leave Grants Pass, but we knew that Kayla's golf was her primary concern in making the move.
