She was preceded in death by mother, Martha Breshears; father, Tommy Gates; sister, Shari Nelson; and brother, Greg Gates.
She is survived by husband, Sam Moses, children, Telia Smith, Candice Gibby, Lucas Gibby;, grandchildren, Haley, Dylan, Skyler, Issac, Kaden, Isabella, Payton, Brooklyn, Sariah, and James Ryan.
Jackie was a strong, bold, and unforgettable woman. She left a lasting impression on everyone who knew her. She will be deeply missed and forever remembered.
The family has chosen to hold no services. Arrangements by Biereley-Hale Funeral Home, Madisonville.
