Jay Rudolph Clark, age 64, of Appleton, passed away on September 12, 2025.
He was born on July 4, 1961, at St. Agnes Hospital in Fond du Lac to the late Jerry J. and Donna Mae (nee Brummond) Clark Sr.
Jay is survived by two brothers, Randy (Deb) Clark and Jerry J. Clark Jr.; a nephew, Ray (Mallorie) Clark; two nieces, Marissa Clark and Morgan (fiancé Mitch Kurtz) Clark; as well as other relatives and friends.
A memorial service in remembrance of Jay will be held on Thursday, September 18th at 3:00 pm at Myrhum-Patten Funeral & Cremation Service (15 S. Walnut St. Mayville, WI 53050). The family will greet relatives and friends at the funeral home on Thursday from 1:00 pm until the time of service at 3:00 pm. Burial of Jay’s cremated remains will be in Graceland Cemetery in Mayville.
