Survived by three brothers, William Samuel (Evelyn) Collinsworth, Fred (Brenda) Collinsworth both of Salyersville, Kentucky, Lloyd (Barbara) Collinsworth of Adrian, Michigan, two sisters, Kathryn (Roger) Phillips, Janie Chism both of Winchester, Kentucky, with a host of nephews, and nieces.
In addition to his parents, Lacy is preceded in death by his wife, Roncie Howard Collinsworth, one brother, Paul B. Collinsworth, four sisters, Hazel Lemaster, Helen Lemaster, Carla Sue Adams, and Garnett Lemaster.
Funeral services will be conducted at 11:00 A.M. Wednesday, October 15, 2025 from the Magoffin County Funeral Home Chapel with William Jenkins and J.R. Owens officiating. Burial will follow in the Ward Family Cemetery at Brushy, Kentucky. Friends may visit the funeral home Tuesday, October 14, 2025 from 6:00 P.M. till 9:00 P.M. and any time prior to funeral services.
Military Rites will be given by the Salyersville D.A.V. Chapter 15 the day of services.
