Lisa Marie Blake, 45 passed away on 1/25/2025 at 4:44 pm at her home surrounded by loving
family and friends.
She was born on 3/7/1979 to the late Barry Allen Blake and Linda Joan Blake in Arden Hill
Hospital in Goshen, NY. Lisa dedicated her life to bringing others joy, laughter, and inspiration.
When she wasn't spending time dedicating her life to Jesus as her Lord and Savior in her beloved
Family Church, she was volunteering her time to charity and devoting her time to her children.
She loved bird watching, hiking, and escaping into her happy place at the Bashakill Wetlands.
Lisa was pre-deceased by her parents and other cherished family members. She is survived by
five children.
Ashley Chabut and her wife Katrina, Jaylee Chabut and her boyfriend Daniel Dailey III, Braiden
Carney, Ava Centorcilli, and her stepdaughter Katherine Centorcilli.
She is survived by two sisters Kristen Blake and her husband Matthew DiMartino, and Jessica
Welsh. As well as all her nieces and nephews.
Lisa leaves behind a loving husband, Michael Centorcilli.
Lisa had one beautiful grandson, Daniel Dailey the 4 th.
We would like to give a special thank you to her best friend Karen Conklin for taking such
wonderful care of Lisa throughout her entire journey battling cancer.
A visitation will be held on Friday 1/31/2025 from 4-7pm at Applebee McPhillips Funeral Home
130 Highland Ave. Middletown, NY.
A funeral service will be held on Saturday 2/1/2025 at 10am at the Family Church 510 M and M
Rd Middletown, NY.
Burial will be at 1130am Saturday at Hillside Cemetery 50 Mulberry St Middletown, NY.
