She was born June 17, 1974 in Somerset, KY. She was a homemaker.
Melissa is survived by son, Anthony Phelps of Cookeville; daughters, Destiny Phelps of Somerset, KY and Whitney Phelps of Monticello, KY; sister, Elizabeth Turpin and three grandchildren.
She was preceded in death by her parents, Mack and Georgia Worley Carrender.
