Samuel Jay Bake, born on February 14, 1949, passed away on January 30, 2026 at the age of 76 at Gulfside Hospice in New Port Richey, Fl.


He was born in Carrollton, Ohio to Samuel and Lucille Bake. He grew up in Carrollton, went to school there and Stark Technical College in Canton, Ohio.

He served in the U.S. Navy from 1967-1971 in Vietnam.


Sam worked for BellSouth for 30 years, retiring in 2006. He served as president for his Union-local 3105. He was a Eucharistic minister and member of St. Thomas Aquinas Catholic Church and R.C.I.A.


He married Ann Ostarchvic on May 10, 1975. They were married for 50 years.

Sam loved all kinds of sport’s especially cheering his favorite team on, the Pittsburgh Steelers.


He is survived by his wife, Ann, daughter Andrea Bouchard (husband Josh). He was blessed with 2 grandchildren, Mae and Grady. They were the apple of his eye and he enjoyed spending time with them. He was a proud father, grandfather and devoted husband. He had a heart of gold, was the best listener, and he always found a way to see the best in people. When he wasn’t laughing with his family and friends, he was listening to his favorite music. Sam loved traveling to Las Vegas and cruising in Europe with his wife. His love for his family and friends was the biggest gift in life.


He leaves behind 2 sisters, Sue McFerren and Judy Lowdermilk, as well as many nieces and nephews He was preceded in death by his brother, Robert Miller. 


A memorial mass will be held March 3, 2026, at 11:00AM, at St. Thomas Aquinas Catholic Church. A celebration of life will be in the parish center following the mass.
