Arthur Joel FORMAN

FORMAN, Arthur Joel (M.D.) age 72, of Tampa, died Monday, March 10, 2014. Dr. Forman was born in Mt. Vernon, NY, and came to Tampa 38 years ago. He served in the Public Health Service Corps of the uniformed services and later practiced psychiatry for many years in private practice. His patients knew him as not only a competent physician but as a friend who took a genuine interest in their well being and care. He served in many professional settings and was former chief of psychiatry at St. Joseph's Hospital and was a member of the Hillsborough County Medical Association. He was a member of Congregation Rodeph Sho-lom. Survivors include his beloved wife of over 46 years, Susan; his sons & daughters-in-law, Dr. Seth and Hannah Forman, Dr. Jonathan and Jessica Forman, and Dr. Howard and Shira Forman; his cherished grandchildren, Abigail, Jadyn, Samuel, Asher, Ari, Gabriel and Eli; sister and brother-in-law, Brenda & Ira Schloss; and his sister-in-law and brother-in-law, Elaine & Richard Desatnick. His memory will endure as a blessing by his family, patients & friends. Funeral services will be held today, (Tuesday) at 2 pm, at Congregation Rodeph Sholom, at 2713 Bayshore Blvd., Tampa. Interment will follow at Beth Israel Cemetery. In lieu of flowers, memorial contributions may be made to Congregation Rodeph Sholom or the Hillel Academy of Tampa.
