FORM, George, 88, of Iselin, N.J., died peacefully at his home on Sunday morning, September 26, 2010. He was the beloved husband of Miriam "Mitzie" Form, of blessed memory, who predeceased him. George was born and raised in Newark, N.J. He was the son of Gussie Bernstein and Marcus Form. He was predeceased by his siblings, Blanche Buddish, Helene Pulsinelle-Schoenwissner, Harry Form, Bernard Form and Charlotte Ebert. He graduated from South Side High School and attended Seaton Hall University. He and his wife and two sons moved to Iselin, N.J. in 1953, where he and his wife raised their family. George was a Staff Sergeant and decorated Army veteran of World War II, serving in the 110th Infantry Regiment of the 28th Division. For his service he was awarded a Bronze Star, three Purple Hearts, as well as the Unit's Presidential and Congressional Citations. George was a charter member of congregation Beth Shalom in Iselin, where he served twice as its president as well as holding several offices and serving on its Board of Directors. His greatest contribution to the congregation was his establishment of the United Synagogue Youth Chapter of the synagogue and directing the programs and activities to encourage the cultural and educational growth of the youth of the synagogue. George was also a member of the Youth Commission of the Northern New Jersey Region of United Synagogue, as well as a member of the United Synagogue of America's National Youth Commission. He served as an advisor of youth activities in several Jewish communities including Iselin and Flemington, N.J. George was employed for many years at Webcraft Printing Company in North Brunswick. He retired from there in 1986. From that point on he and his wife, Mitzie, until her death, enjoyed retired life. George Form is survived by family and many close friends. George's two sons, Mark Form and his wife, Mary Betty Watkins Form, of Colonial Heights, Va., and Mark's son, Aaron Form of Richmond, Va., their daughter, Kathryn Korb and husband, Dave, and their children, Alex and Sam also of Colonial Heights; Bruce Form and his wife, Mira Katz Form, of Highland Park and their children, Karen Form of Queens, N.Y., Ari Form and Becca Bray of Brooklyn, N.Y., and Zev Form of Highland Park; family relations, Gloria and David Lederman and Myrna and Barry Axelrod. Of his many friends are Mary Burnas, Phil Schreiber, Morris Heller and Larry Steinberg. Funeral services will be held 10:30 a.m. on Tuesday, at Congregation Neve Shalom in Metuchen, N.J., followed by burial at Mt. Lebanon Cemetery in Iselin, N.J. In lieu of flowers, contributions can be made to the Mitzie Form Scholarship Fund, c/o Woodbridge Township Education Association, 34 Green St., Woodbridge, N.J. 07095. My deepest sympathy to all of the Form Family. George and I worked together for 12 years at Webcraft. I was his secretary for years and then became a buyer. We had so many laughs and years of memories. He was so knowledgable, fun and always had time to help in any way. The memories are still vivd and I will never forget the love that he and Mitzi had for eachother. May he rest in peace. Sincerely, Bonnie Adams
