1937 - 2021
Jack Louis Cummins, age 83, passed peacefully on May 19, 2021 at Mansfield Hospital. Preceded in death by wife Donna Jean, parents John and Thelma Cummins, sisters Audrey Shiplett and Mary Armentrout. Survived by children, Pam (Greg) Scott, Jackie (Lewis) Mould and John (Bobette) Cummins; grandchildren, Sarah (Eric) Mikesell, Beth (Justin) Koch, Martha York and Matt Smetanko; great grandchildren, Alexandra, Aidan, Henry and Cecilia; brother-in-law, Howard (Lynda) Southall; and sister-in-law, Betty Slark; numerous nieces and nephews. Jack was employed at Columbia Gas of Ohio for 37 years, where he enjoyed playing softball, bowling and golfing with his co-workers. Jack was also Past Worshipful Master of Groveport Lodge #240 F&AM and current member of Groveport Lockbourne Lodge 801. Masonic service will be 5pm Saturday, May 22 with visitation immediately following until 8pm at the Myers-Woodyard Funeral Home, 587 Main St. in Groveport, OH, where funeral will be held Monday, May 24, at 11am. Interment at Obetz Cemetery. In lieu of flowers, donations may be made to St. Jude Children's Research Hospital or Ohio Health Hospice in Jack's memory.

I’m so sad to hear of Jack’s passing. I fondly remember both Jack & Donna at many get togethers with my parents Bill & Barb Sweetman. They were both overly hilarious and such a loving couple. You are all in my prayers Pam, Jackie and John.
