Sally and Family,
My heartfelt condolences to you and to eveyone Jack touched over all these years. Little did we know when we walked the halls of RHS way back in 1965, where life would lead us and what part we would play. You can rejoice in knowing Jack played his part to the fullest but left the stage too soon.

Sally,
I met John when I came to Columbia Hospital in 1993, and then I met you. Through these years I have known your joys, and shared times on vacation, and the road the two of you travelled during John's illness. Your time together has been rich, your memories will be full, and God will comfort your heart now in your time of mourning. My prayers are with you and your family. In sympathy,

I'm very sadden by Jack's untimely death. I value the time we spent together as professional colleagues. John always lead with his heart and as a health professional always focused on the needs of patients and improving the health status of our community. He was a compassionate individual, while always soft spoken, he had a burning passion for others. He will be missed by all who knew him and had the good fortune to work with him.

Bill Petasnick, a friend and colleague.
