Thomas Forma Obituary
Thomas John Forma, 58 years old, died suddenly in his home. Born February 26th, 1955, in Yonkers to the late John and Florence Forma, "Tommy" is survived by his sisters Janice Faia of Port Chester, NY, Geraldine Grey of Boston, MA, and his twin, Felicia Forma of Doylestown, PA, a niece, Felicia Cossifos of New Rochelle, NY, nephew Jay Grey of Worcester, MA, and great niece and nephew Alexis and Justin Cossifos of New Rochelle.
An alumnus of Mt. St. Michael High School, "Tommy" attended The Academy of Aeronautics at La Guardia, and graduated from Embry Riddle Academy in Daytona. He owned Guerra's Auto Body Shop in Yonkers until 1999. A longtime member of The Catholic Slovak Club, he loved the water and enjoyed boating, fishing and scuba diving. He was an avid NASCAR fan and a regular at The Daytona 500.
Memorial Service will be held at Bryn Mawr Chapels, 23 Lockwood Avenue, Yonkers, NY on Thursday, September 19th from 6 - 8PM. No flowers please.

Remembering the good times....like when we travelled around the world and got stuck in Vietnam, or when you would try and trick me and steal my mushrooms off my dinner plate, when you used to drive me to school on your motorcycle or in the Corvette and all the older girls would wonder who that hot older guy was, our trips to Montauk or bowling with you and your friends....you were like my big brother....miss you always!

It's been one month ago today
I found that you had slipped away.
Leaving without a last goodbye,
I want to know the answers why.
Why wasn't I there to hold your hand?

A final hug, a final glance ~

Though family and friends had tried to help.

I am so sorry to hear about Tommy. So many memories of him and all the kids on Morsemere and Greenvale. Just so hard to believe he is gone. My prayers and thoughts to Janice, Geri, and GiGi, and Felicia. I am thinking of you all.

I am so very sad to hear that tommy passed away. We were neighbors on morsemere avenue and spent a lot of time with Tommy. When my mom died he was there for me. My thoughts and prayers go out to the family

I met Tommy when I was 3.5 years old. He and GiGi and I were thick as thieves growing up. I am so saddened by his untimely passing. My deepest condolences to Felicia, Janice, Geri, Felicia Ann and the entire family. May he rest in peace.

I am very sorry to hear of the passing of Tommy. I remember him well from Sacredy Heart grade school and Morsemere Avenue. I am sure you all now have a special guardian angel to watch over you. My prayers are with you and your family.

so many good memories of tommy, but one that i want to share is that when my mom's birthday was coming up and the family still lived in yonkers and tommy had chase st house and the pool, tommy drained it and we both scrubbed the pool and made a great nite for my mom's birthday. tommy fillled the pool, by passing the meter another time canoeing down the river near nj and the first nite we all pulled into a llittle town and we all hit the nearest bar and so in the morning kinda lost contact of who left early and not, well while paddling down the river, see tommy and ray, last name i forget and they were cooking up bacon and eggs and shared with us, not a second went by where they did not think of sharing. having christmas eve at the chase house and cooking it all, traditional polish dinner, tommy and his whole family were there and was so great. i will always miss you tommy. your friend billy.

I don't even know if words can convey how deeply Tom's passing has affected me. So many good memories come to mind from those childhood years. I remember being comforted by a hug from him often. He always seemed to understand when u needed one. I remember his hearty laugh and his big smile. I remember being devastated when he told me there was no Santa Claus!!!! I know how difficult a time this is for your family - try to let the love of your family and friends and the good memories bring you comfort. You are in my thoughts and prayers and I will always remember Tom with a smile!!
