Clara Catherine Forma 89 was born on November 16, 1935, in the village of Sloan, N.Y., the third child of the late Genevieve & Vincent Januchowski. Clare was the dear sister of her late siblings Vincent Januchowski, Genevieve Bialoblocki and Ronald Januchowski.
Clare was the wife of late John Forma and is survived by her loving sons, John Forma (Tom Marko), Lawrence Forma (Cindy Forma) and Kevin Forma (Matt Randle), granddaughters, Jessica Forma, Sarah Forma and Laura Forma and great grandchildren Damian, Antonio and Aubrey.
Clare was an inspiration and support for many cousins, nieces, nephews and friends. Clare personified love, compassion and the enjoyment of life and will be profoundly missed and loved forever in our memories.

I am so sad to hear Clare has passed. I worked with her for years until she retired and I always considered her a good friend. She had such a great sense of humor, and she was very down to earth, and she always gave good advice. I always felt like I could talk to her about anything. She was like a mother/aunt/sister to me. We kind of lost touch when she moved away and I missed seeing her. I miss her and I will miss her. My condolences to all of her sons and to all of her family!
