He was a cable splicer for South Central Bell Telephone Company and attended the First Presbyterian Church of Pikeville.
Daniel was born in Pikeville, Kentucky on July 13, 1945, the son of the late John Dils Forsyth Sr. and Jean Clair Moon Forsyth.
Along with his parents, he was preceded in death by three brothers, Augustus Preston "Buddy" Forsyth, John Dils Forsyth Jr., James Price "Jim" Forsyth and three sisters, Linda Grace Forsyth, Sylvia Marie Forsyth and Rosa Lee Forsyth.
He is survived by his wife, Jeanetta Karen Thomas Forsyth and four daughters, Salena Sims (Brian) of Winchester, Kentucky, Tina Forsyth of Pikeville, Kentucky, Danette Biliter of Pikeville, Kentucky and Angela Gross of Pikeville, Kentucky.
He is also survived by twelve grandchildren and nineteen great-grandchildren.
Private Services will be held.
