Candace Michelle Pembo Jacobi

Candace Michelle Pembo Jacobi, 35 years old, of New Orleans, Louisiana, died Monday, October 27, 2014 of complications from kidney disease, at her home in Arabi, Louisiana. Candace was born on June 12, 1979 in New Orleans, Louisiana. Candace was formerly a Pediatric Transplant Coordinator at Tulane Medical Center, where she was a shining light and a role model to many young transplant patients and their families dealing with renal disease, kidney failure, tranplantation and dialysis. As a pediatric patient at Tulane Medical Center for many years, she always told her doctors she would be their nurse when she grew up and it was her dream to give something back to families dealing with kidney disease. Candace received her first transplant from her father at the age of 14 and her second transplant from a wonderfully kind co-worker 6 years ago. She served on the Board of the National Kidney Foundation of Louisiana Walk for many years. Candace is survived by her beloved son, Caleb Reese Jacobi; her mother, Dawn Neese Pembo; her father, Chris Pembo, Jr.; and her brother, Brandon Christopher Pembo. Friends and loved ones will be received Friday, October 31, 2014 at 9:00am at St. Paul Lutheran Church followed by a church service at 11:00am.
