Funeral: 10 a.m. Tuesday, July 8, 2003, in the New Faith Baptist Church in Annetta. Burial: Annetta Cemetery.
David was born March 8, 1967 in Fort Worth. He was a devoted husband, father, son, brother and a friend to everyone. He attended Chapel Creek Fellowship. A special thanks to Dr. Debra Prow and staff for their loving care
Survivors: Wife, Tina Kay Sample, son, Jonathan Sample, father, Reuben Sample all of Fort Worth, mother, Nancy Holznagle of Chama, N.M., brothers, Les Sample of Fort Worth, Charles Sample of Lebanon, MO, Mike Sample of Lebanon, Mo., father and mother-in-law, James and Nelda McGraw of Annetta; and many aunts, uncles, nieces and nephews.

I am so sorry for your families loss. I had no idea David had cancer. The last time I saw him at Striplings he was his wonderful polite self. I know he will be missed by all. Take care of yourself, Tina and Jonathan.

David sent us many cards to inspire and encourage us. my husband had bladder cancer under went a twelve hour surgery David sent cards in the hospital, also after mickey got home even though David was fighting his own battle. God give peace and comfort to his family, The Mcdonald's
