John Mark Crates, M.D. PLANO--Dr. John M. Crates passed away Saturday, Dec. 28, 2019, surrounded by family. MEMORIAL SERVICE: 3 p.m. Thursday, Jan. 9, at Highland Park United Methodist Church. John grew up in Chattanooga, Tenn., where he graduated from the Bright School and the McCallie School. He was passionate about sports, excelling in both varsity football and soccer. He earned his undergraduate degree at Southern Methodist University and went on to get his medical degree at the University of Tennessee in Memphis, after which he completed a Foot and Ankle Surgery Fellowship at the Campbell Clinic. For more than 21 years, Dr. Crates was a valued physician at Plano Orthopedic & Sports Medicine Center, providing outstanding compassionate care for his patients. He was a member of the American Orthopedic Foot & Ankle Society and a fellow of the Academy of Orthopedic Surgeons. He previously served as vice chief of Orthopedics at the Medical Center of Plano, as well as served on the executive committee at Baylor Frisco Medical Center, where he performed surgery. He was also recognized by The National Registry of Who's Who, named by D Magazine for "Best Doctors in Dallas" and honored as a "Super Doctor" by Texas Monthly. Dr. Crates was foot and ankle consultant for the Texas Rangers Baseball Club since 2004, foot and ankle consultant and assistant team physician for the Frisco Rough Riders since 2003, and foot and ankle consultant and assistant team physician for the Dallas Stars from 2005-2008. Dr. Crates was also the founder and CEO of Foot Innovations, a research and development and consulting company for orthopedic-related products. John was a talented musician who played acoustic guitar and bass in bands in high school, college and medical school. He loved golfing, fishing, hunting, grilling for his friends and watching a good sports game, but the biggest joy in his life was his family - his loving wife, Angela, and their daughters, Chandler, Cameron and Campbell. Committed to his children's schools, John served on the board of trustees at The Lamplighter School and for many years as the team physician for the Greenhill School football team. He was also proud to serve on the University of Texas Family Leadership Council where he was helping to launch an internship program. Others enjoyed having John around and appreciated his kind heart. He was the person to go to when you needed a true friend. His witty sense of humor and approachability made others laugh and feel at ease. John was preceded in death by his mother, Gladys Hayes Crates; his father, Robert Cameron Crates; and his brother, Robert Bruce Crates. SURVIVORS: He is survived by his wife of 26 years, Angela Tilley Crates; their daughters, Chandler, Cameron and Campbell Crates; uncle, Daniel M. (Elise) Crates; cousin, Cameron C. Matthews; sisters-in-law, Marisa Tilley Hammond, Lisa Tilley (Michael) Anderson; brother-in-law, Rice Matthews (Michelle) Tilley III; nieces, Lucy and Samantha Leonard and Morgan Tilley; and nephews, Tripp and Parker Hammond and Rice Tilley IV.

I was saddened to hear of Dr. Crate´s passing. I loved Dr. Crates. He saved my leg after a very severe accident. But most importantly, he saved me. I was a single divorcee when Dr. Crates performed 3 surgeries over several months to save my leg. During one post-surgical visit, he noticed that I was depressed, even though I never vocalized my feelings. When he noticed my depression he closed the examination room door and talked with me for more than 20 minutes in a successful effort to improve my mental state. He told me a story of how he "worked through" a similar period in his life and shared some very personal experiences. All of this was his way of trying to make me feel better. It worked. I don´t have the best words to express my gratitude to him. After I was finished with my surgeries, I wrote him a personal letter telling him how much he did for me and how much I appreciated him. Several months later for my final follow-up visit, he told me that he kept my letter in his desk and read it whenever he had a difficult day.
I realize that probably nobody will read my comments here because it is nearly 5 years after his death, but because I moved to Las Vegas I only learned if his passing now.
I hope that if his wife, children or other close family or friends might read this and count me among the many people who knew his kind, thoughtful and special Dr. Crates was. No doubt that his special warmth touched many more people. I know he is in heaven. Thank you Dr. Crates.

r. Crates was an awesome and highly-informed physician. I was a patient of his and I’d go in with 1 idea and come out with a better plan. I am devastated he is no longer with us. He was undoubtedly one of my ever favorite doctors.

Dr. Crates was a gifted doctor and surgeon, but more importantly he was an exceptional person. Although he was my doctor and surgeon, he invested in me as a friend and brother in Christ. He showed genuine concern about me as a person,as well as how I healed after surgery. One could tell from the pictures and notes on the office walls from many, many patients how he touched and changed lives in a very special way. I loved going to see Dr. Crates and will miss him dearly until we are reunited once again in the presence of our heavenly Father.

Our son Jantz had Duchenne Muscular Dystrophy & Dr. Crates was his Dr. when he broke his ankle. Dr. Crates quickly won our family over in the ER with his kindness and professionalism shown towards our family. He will be missed here in Plano.

Dr. Crates' was a man of knowledge,expertise and integrity. I came to him with a toe/foot issue that needed saving and restoring. He was professional and personal as he worked to save my toe. When an open wound caused a second surgery, he was quick to take care and save my toe. In the end, he did 3 surgeries on 1 foot in 1 l/2 years. I'm forever thankful for his knowledge and surgical help, but his gracious and humble spirit I will remember too. He always told me God healed me, not himself. I'm praying for his family. What an amazing man, doctor, and friend to so many.

I just learned of Dr Crates passing today. In 2010 Dr Crates performed a very complicated operation on my damaged ankle. He was so caring and forthright with me about the surgery. To this day I am so grateful for his skill and compassion. One dr wanted to amputate my foot. Today I am doing 99% of what I did before my ankle problem My sincere condolences to the family may the Peace of Christ envelop you all.

Dr. Crates will forever be in my heart. He helped me after my first surgery was botched. His smile and his compassion won me over. I new from our first visit he would help me regain my health. He new I was nervous so he made it a point to visit with me personally after my surgery. My family and I will always remember him for the great man he was. I pray for his family and friends. OMG, I'm saddened Dr Crates was the Best Doctor ever he tried so hard to correct a surgery that another doctor performed he was determined, kind and had the absolutely best bed side manner We will always remember him and the smiley face he placed on my toe before surgery.

Devastating news. Dr John repaired both of my heel cords and Achilles. Referred by Dr Hunter, I thought my active life was over! During my initial consult, a famous Texas Ranger was coming out before me. I said to my wife, Dang, I may have the same Dr as (blank), my chest swelling. My wife said, just as Dr Crates turned to see us, Yea, think how that makes (blank) feel. Dr Crates busted out laughing and we became fast friends. My problems required skill I believe only an incredible surgeon could repair. He prayed with me and gave me back my mobility!
