Carole Cleo Slade Church passed peacefully into the next life on November 16, 2025, surrounded by her children, mother, and loving husband. Born April 28, 1972, to Jared and Deborah Slade, Carole was the oldest of ten children. She spent her childhood in rural Colorado helping on the family farm, making memories with siblings, cousins, and friends, and growing her testimony of Jesus Christ. Carole was a faithful member of the Church of Jesus Christ of Latter-day Saints all her life. The family moved to Utah during her teen years where she graduated from American Fork High School, and attended Ricks College in Rexburg, Idaho. Carole was a dedicated administrative professional in many work environments that gave her opportunities like learning to fly and solo an airplane, and spend more time at home to raise her family which was her greatest accomplishment. She worked for K & P Plumbing and North Sanpete School District as a financial assistant. These jobs gave her the opportunity to make special friendships and meet more people to love.

Carole met the love of her life Brandon after they were set up on a blind date by their brothers Ryan and Cody. They were sealed for time and all eternity on October 9, 1998, in the Manti Utah Temple. In her words their marriage was “the greatest love story ever.” One of teamwork, service, and true happiness with Jesus Christ at the center of their marriage and family. They welcomed five incredible children into their family: Bo, Victoria, Halle, Sadie, and Emily. Carole and Brandon recently celebrated their 27th wedding anniversary in October. Raising her family was Carole’s passion and greatest joy. She taught her children to love art and music, to cook, sew, read, and most importantly how to love and have faith in their Savior and His gospel. Carole’s home is one where everyone is comfortable and can feel the presence of the Holy Spirit.

One of the things Carole most loved was going on adventures with her family. On her road through life her greatest treasures were the people and friends she met along the way. Carole was the greatest of friends. She had a way of making everyone around her feel special and loved. Her testimony, love of God, and love of people radiated. Her joy in the Savior and gospel of Jesus Christ was apparent in everything she did. She truly was a woman for such a time as this.

We love you Carole and are thankful for your light, example, and kindness to each of us.

She is preceded in death by her father, Jared Slade, and sister-in-law, Shara Church.

She is survived by her adoring husband, Brandon; five children, Bo (Jada) Church, Victoria (Hayden) Smith, Halle Church, Sadie Church (Gavin Iund), and Emily Church; two grandsons, Kolter Church and Tucker Smith; mother, Deborah Slade; nine siblings, Kelli (Robert) Ostler, Rebecca Slade, Ryan (Bonnie) Slade, Joel (Maria) Slade, Alicia (Jeffrey) Hale, Stephanie (Kolby) Blackner, Joshua (Chelsea) Slade, Anjali Slade, and Micah (Morgan) Slade; in-laws Dennis and Rhonda Church and six brothers- and sisters-in-law, Cherish (Devron) Larson, Cody (Shannon) Church, Camie (Donny) Worley, Kade (Ciara) Church, Lacey (Wayne) DeMille, and Tyson (Karissa) Church; and several nieces, nephews, and great-nieces and nephews.

Funeral services will be held Friday, November 21, 2025 at 2:00 p.m. in the Mt. Pleasant North Stake Center (461 N 300 W) with a visitation from 11:00 a.m. to 1:30 p.m. prior to services at the Stake Center. Interment in the Fairview City Cemetery. 

Per Carole’s request “Everyone please bring a penny to put in a jar to remind my family that “In God we trust”.“ The family is collecting memories and pictures of Carole from her friends and loved ones. If you could please bring a page with a picture and a story or memory about how Carole gave you hope or brought you closer to your savior Jesus Christ it would be appreciated.
