Dr. Helen F. Ptak of Middletown, NY, entered into rest on Wednesday, February 26, 2014 in Middletown Park Rehabilitation, Middletown, NY. She was 86. The daughter of the late Anthony & Genevieve Reiss Ptak, she was born on January 29, 1928 in Wawayanda, NY. Survivors include her 2 sister-in-laws, Suzanne Ptak of Milford, DE and Loretta Ptak of Middletown, NY; 5 nieces, Catherine Faller, Pamela Meikle, Patricia LaManna, Dr. Antoinette Ptak & Lenore Sullivan; 5 nephews, Anthony, Michael, Robert, William and Raymond Ptak; many great nieces, great nephews, great great nieces, great great nephews and cousins. She was predeceased by brothers, Walter, Henry and Dr. Leonard Ptak; Sister-in-law, Frances Ptak; nephew, Walter Ptak Jr. and niece, Linda Licardi. Helen was very proud of her accomplishments, which included working her way up from nursing to obtaining a PhD in Preventive Medicine at Ohio State University. She taught at the University of New York (Buffalo, NY), Ohio State, the Texas Woman's University, the University of Texas and the University of Southern Mississippi. Helen loved her family and she will be dearly missed by all who knew her. Visitation will be held from 4-7PM on Wednesday, March 5th at T.S. Purta Funeral Home, 690 County Route# 1, Pine Island, NY 10969. Funeral mass will be held 11:00AM Thursday, March 6th at St. Stanislaus Church, Pine Island, NY. Burial will be in St. Stanislaus Cemetery, Pine Island. Memorial Contributions may be made to St. Joseph's Catholic Church, 149 Cottage St., Middletown, NY 10940 or Catholic Charities, Office of Development, 11th Floor, 1011 First Avenue, New York, NY 10022.
