She was born November 10, 1955, in Oklahoma City, Oklahoma, the daughter of Freeman Coy Duncan and Johnnie Mae (Whisenhunt) Duncan.
Juanita worked as a caregiver. She was a member of Lindsey Chapel Baptist Church. She enjoyed collecting antiques, frequenting garage sales, and crafting. Most of all, she delighted in the company of her family and friends.
Survivors include her son, Leslie Phillips of Checotah; her grandchildren, Rebekah Gourley and her husband, Seth of Checotah, Samantha Phillips of Checotah, Skyler Walker and her husband, Shane of Checotah, Noah Phillips and his wife, Alicia Casinger of Checotah and Matthew Phillips of Checotah; five great grandchildren, Dawson, Kash, Elizabeth, Hudson and Marlow; her siblings, Johnny Duncan and his wife, Julie of Eufaula, Geraldine "Pudd" Cannon and her husband, Richard of Checotah, Daphnie "Duck" and her husband, Matt of Muskogee and Roger "Dick" Duncan of Longtown.
Her parents, Freeman and Johnnie Duncan; her siblings, Jimmy "Peewee" Duncan, Debra "Joan" Degraffenreid, Jeanette Wheeler, Janet "Little" Moore, and Betty Degraffenreid, preceded her in death.
