Passed away on Wednesday, October 8, 2025 at the age of 75. Beloved son of the late Andrew and Alice Poliak. Further survived by cousins, other relatives and friends.
Visitation to be held on Thursday, October 16, 2025 at CHURCH AND CHAPEL FUNERAL HOME, 7626 W. Greenfield Ave. from 11AM-12PM with a Funeral Service at 12PM. In lieu of flowers memorials to The Milwaukee Transit Archives and Museum appreciated.
Tom was a longtime employee of Harwood Place Senior Living, an avid photographer, and contributor to books and racing. He enjoyed anything with the Milwaukee Transit Authority. Tom had a corny sense of humor and enjoyed a good pun. Special thank you to the staff at Mitchell Manor for their care of Tom.
