EAST PEORIA - Jazmine Mae Ivy, 25, of East Peoria passed away at 4:02 a.m. Sunday, September 17, 2017, in Peoria.
She was born on June 5, 1992, in Peoria to Sherri Button.
Surviving are her mother of Peoria; her father, Devan Lyons of Peoria; her daughter, Avaya Skye Renee Ivy of Peoria; grandparents, Juanita and Terry Belcher, Rev. Alphonso Lyons, Gwendolyn C. Lyons and Ella Ivy; great-grandmother, Dorothy "Nana" Green; three sisters, Britny Button, Shaylyn Lyons and Cheryl Lyons; one brother, Dallas Lyons; her biological father, Lamarr Ivy; and and several aunts, uncles and cousins.
She was preceded in death by her grandfather, George Button.
Jazmine had worked in home health care in the area.
She enjoyed doing photography, crafting, singing and dancing.
Her memorial service will be at 11 a.m. Saturday, September 23, 2017, at Mt. Zion Baptist Church, 305 S. Madison Park Terrace, Peoria, IL 61605. Her grandfather, the Rev. Alphonso Lyons will officiate. Visitation will be from 9:30 to 11 a.m. at the church.
Arrangements have been entrusted to Preston-Hanley Funeral Homes & Crematory in Creve Coeur.
Memorial contributions may be given to the family for an educational fund established for Avaya.
