Chase Patrick Smith Peoria - Peoria- Chase Patrick Smith M.D., age 28, of Peoria, formerly Orrville, OH, passed away unexpectedly on Sunday, February 12, 2023.

Born April 7, 1994 in Wooster, OH, a son to Richard William (Debra) Smith Jr. Chase was raised in Ohio and educated in Lufkin, Texas, graduating from Lufkin High School with the Class of 2013. After high school, he attended The Ohio State University where he obtained his microbiology degree and went on to enroll at the University of Illinois College of Medicine of Peoria, graduating with the Class of 2022. On January 15, 2022, he married Ashley Elizabeth Bertino in Peoria and together they welcomed their daughter, Adelaide, and made their home in Peoria. Chase was an avid Ohio State fan who loved riding his bike, kayaking and anything else outdoors. He was a beloved son, husband, father, and brother who will be remembered for his deep love of family.

Survivors include his parents Richard (Debra) Smith of Bardstown, KY; wife Ashley Smith of Peoria; one daughter, Adelaide Francesca Smith of Peoria; three brothers: Richard William (Florence Wu) Smith III of Denver, CO, Kyle Christopher (Lisan) Smith of Champaign, IL, and Cameron Lee (Brittany) Smith of Gilberts, IL; one nephew Zachariah Smith and one niece Eleanor Smith; and paternal grandparents Rev. Richard (Betty) Smith, Sr. of Wooster, OH.

Chase was preceded in death by his maternal grandparents Dencil (Joyce) Smith.

Visitation will be held at Wright & Salmon Mortuary, 2416 N. North Street, in Peoria on Saturday, February 18, 2023 from 10:00 AM until time of funeral service at 12:00 PM. Pastor Mark Nelson will officiate, and cremation rites will be accorded following services.

Preferred memorials in Chase's memory may be made to the American Foundation for Suicide Prevention via www.AFSP.org or to the family to help set up a college fund for his daughter Adelaide via https://gofund.me/8b7ad0ba.

I am so sorry for your loss. I didn't get the opportunity to meet Chase in person, but I spoke with him frequently. He was always so nice when he called into PALs. We'll miss hearing from him. He was one of my favorites.

Chase was one of the first friends I made during undergrad. We went on several trips over the years, and spent many weekends hosting gatherings at each other´s college houses. Chase is a part of many fond memories from my college years. I lost touch with him since OSU, but I remember him as a kind, funny, and intelligent person. I´m saddened and so sorry to hear of his passing, and I wish your family all the best.

I met Chase as a third-year during my first clerkship in OBGYN, and we became good friends since then. As fourth-years, we were co-presidents of the Diagnostic Radiology Student Interest Group where I had the fortunate opportunity to see his creativity and passion for recruiting other students to our favorite specialty. We even sat at the same table for Match Day, and I´ll never forget how excited he was when he matched into one of his top-ranked programs. I will miss him greatly. My deepest condolences to his family.

Chase was an amazing person. Great at ping pong and always humorous. He was honest - the kind of person who did the right thing even when he didn't have to. He cared deeply about his work and his relationships. We admired that about him. We pray for peace. For him, and for his family.

I worked with Chase during the first 2 weeks of my residency. Even during that short period of time I was able to see how smart and kind Chase is. He helped and taught me a lot during that period. He will always be in our memories. A heartfelt condolences to Ashley and all of Chase´s loved ones!

Had the honor of getting to know Chase while rotating through pediatrics. He was such a kind, thoughtful person interacting with patients and families-an excellent physician all-around. I specifically remember how he enjoyed talking about his daughter in between patients, and his eyes would light up when doing so.

I knew Chase as a medical student and then as one of our interns. His sense of humor, compassion, and kind heart were truly gifts to all of us. While we mourn his passing that came much too early, I know that all of us here at UICOMP, his colleagues and patients, are better since we knew him.

I am so sorry for your loss. Chase and I spent a lot of time together during our third year internal medicine rotation. We played numerous rounds of ping pong and pool. I was terrible at both games, but I always had a good time with Chase.
