Deborah Ann Hazelwood, 75, died Thursday, November 6, 2025.

Debbie was born September 2, 1950 in Ft. Wayne, Indiana, daughter of Jack and Geraldine Hill. She attended Seneca High School and was a graduate of Atherton High School and Western Kentucky University. She was a teacher and retired from Luhr Elementary School.

Survivors include her sons, Clay Armstrong and Chris (Charity) Chambers; husband, Tim Hazelwood; grandchildren, Natori and Jenesis Armstrong, Jaedyn Chapple (Ezra) and Corbyn Chambers; and sister, Sandy Baird (Larry).

Visitation is 3-7pm Tuesday, November 11, 2025 at Fern Creek Funeral Home, 5406 Bardstown Road, Louisville, Kentucky.

In lieu of flowers, please consider contributions to WHAS Crusade for Children.

It is people like Debbie who have made it possible for the Crusade to help children with healthcare, educational and emotional wellness needs for over 70 years. Thank you for honoring her memory by remembering the WHAS Crusade for Children. Debbie's legacy will live on for years to come through the lives of children who are helped by the Crusade. We will keep you in our thoughts and prayers.
