Hazel was born in Ritner, Kentucky, on March 22, 1951, the daughter of the late Samuel Tate and Nancy Jane (Jones) Phillips. She is also survived by grandchildren Matt, Zack, Tasha, Cassie, Jordan, Erica, Sam, Tadessa, Shadara, and Maria; 21 great-grandchildren; one great-great-grandchild, Mia; special niece, Barb Gregory; special friends, Rhonda Kendziorski, Vicky Coffey, and Julie Helton; and a host of family and friends of which are to numerous to name.

Funeral services will be held at 1:00 p.m. on Wednesday, February 18, 2026, at McCreary County Funeral Home with Bro. Eddie King and her grandson, Zack Troxell, officiating. Burial will follow in the Rock Springs Cemetery. The family will receive friends after 11:00 a.m. on Wednesday at the Funeral Home.
