Mike was born on Saturday, March 31, 1951 in Shreveport, LA and passed away on Monday, October 6, 2025 at home with family by his side in Jonesville, LA. Mike was widely respected for his strong work ethic and remarkable craftsmanship as a gunsmith. His natural mechanical talent, attention to detail, and marksmanship earned him the admiration of friends, colleagues, and customers alike. Above all, Mike's love for his God, Jehovah, was the guiding force in his life. He dedicated his life to serving Jehovah in the fall of 1972. His faith was evident in the way he lived, in how he led his family, and it remained at the center of everything he did.
He is lovingly remembered by his wife, Jan Holland Forman of Jonesville, LA with whom he shared 53 years of marriage; son, Chris Forman of Oak Grove, LA; son, Bryan Forman and wife Michelle of Liddieville, LA. He will be dearly missed by his four grandchildren: Kaine Forman, Abby Fisher, Zack Forman, Gracie Forman and by his many nieces and nephews and extended family.
Mike was preceded in death by his parents, J.N. and Velma Forman; his sister, Sue Holland; and brother-in-law, Lloyd Holland.
Pallbearers will be Kaine Forman, Zack Forman, Wayne Holland Jr, Jason Holland, Ashton Holland, Payton Holland, Matt Salter, and Bill Holland.
Honorary pallbearers are Wayne Holland and Louis Riley.
