Jeremy is survived by his mother, Rita Wells; father, Michael (Hannah) Smith; brother, Sean (Chelsea) Smith; two half-brothers, Bryan and Jason Smith; grandmother, Donna Wells; uncles, Rick (Amy) Wells, Jeff (Barb) Wells and David Smith; aunt, Amy (Bill) Baker and several cousins
He was preceded in death by his grandparents, Donald R. Wells and Ralph and Barbara Smith and aunt, Lynne Kemerly.

Jeremy was a good friend to my son Shawn. They enjoyed going out to eat and grilling out. He will be missed by so many. Prayers for all may he find peace in heaven.
