David is survived by his wife Mandy; one son, Jason Phelps of Lebanon, MO and one daughter, Sabrina Long and her husband Rob of Camdenton, MO; three sisters, Hazel Jones and her husband Lee of Garden City, Kansas, Judy Shepard and her husband Al of Springfield, MO and Sarah Ann Hamilton of Richland, MO; three grandchildren, Ethan and Sierra Walters and Madison Phelps; other relatives and many dear friends.
He is preceded in death by his parents and one brother, Everett Lee Green.
Memorial Graveside service for David will be held at Missouri Veterans Cemetery, in Waynesville, Missouri, at a later date.
His wishes were to be cremated under direction of Shadel's Colonial Chapel.
